Home wound care covers the assessment, cleaning and dressing of acute, chronic and surgical wounds in the patient’s own home. A specialized nurse visits your home, evaluates the condition of the wound, applies appropriate care with aseptic technique, and monitors the healing process in coordination with the treating physician.
Who It’s For
Ideal for patients recovering from surgery, people with chronic wounds such as pressure ulcers or bedsores, diabetic ulcers, and wounds that require regular care. It serves bedbound patients, elderly people with limited mobility, and anyone who needs ongoing nursing follow-up of a wound at home.
What It Includes
- Wound assessment: Evaluating the type, extent and condition of the wound, along with any signs that require attention.
- Cleaning & dressing: Cleaning and changing dressings with sterile single-use materials and aseptic technique, following the physician’s instructions.
- Healing monitoring: Systematic monitoring of the healing process and timely updates to the treating physician about any change.
Safety & Procedure
Care is provided by a certified nurse following all hygiene, antisepsis and safety standards. The care plan is always defined and adjusted in collaboration with the treating physician, who directs the therapeutic management of the wound.
